What this symptom means clinically
Abdominal sagging is an outward sign of altered abdominal volume or abdominal-wall support. Common mechanisms include body-fat redistribution, intra-abdominal fluid accumulation, organ enlargement, or abdominal-wall muscle atrophy. These changes make the abdomen protrude or sag downward and alter contour and palpation feel. Endocrine disease can indirectly cause similar changes by affecting metabolism and fat distribution.

Because cats are often more slender, muscle loss or centralized fat in older individuals can make the abdomen look saggy; abdominal fluid or organ changes can also markedly alter contour.
